BATTLEFIELD MEDICINE / DALARGIN

TARGET SECONDARY
ORGAN INJURY.

Dalargin is a synthetic enkephalin analogue with opioid-receptor activity. We are testing its rationale for limiting secondary organ injury after trauma. Human hospital observations make it a priority for translational diligence. Field delivery and opioid safety remain unresolved.

01 / HUMAN EVIDENCE

104 severe-trauma patients

A 2023 prospective hospital study compared 38 patients receiving Dalargin with 66 standard-care controls. Reported ARDS incidence was 5.3% versus 21.2%; stage-1 acute kidney injury was 2.6% versus 19.7%. These are observed group rates, not established causal reductions.

02 / STUDY QUALITY

Randomization and masking are not reported

The full 2023 methods describe a single-center prospective comparison with unequal groups. They do not report randomized assignment or blinding. Mortality was 21.1% versus 25.8%, without a significant difference. The study excluded patients with high vasoactive-support requirements and major soft-tissue crush injury, limiting battlefield generalization.

03 / TRANSLATION

Timing and physiology decide

Older hemorrhagic-shock experiments report timing-dependent benefit. A separate 1989 traumatic-shock study found worsened survival in rats. The program must resolve hemodynamics, dose, timing and resuscitation context before field translation.

04 / FIELD PRODUCT

Delivery for the care pathway

Evaluate stability, onset, controllable exposure and compatibility with resuscitation. Hospital infusion findings do not validate a battlefield formulation. Establish the product profile with trauma specialists and qualified laboratories.

05 / OPIOID PHARMACOLOGY

Abuse liability requires direct testing

Dalargin is an opioid peptide, not an established non-addictive alternative. Historical animal work reports limited brain penetration of unmodified peptide, while delivery technologies can increase central effects. Assess receptor activity, CNS exposure, respiratory and hemodynamic effects, dependence and abuse potential for the actual candidate formulation.

06 / U.S. REGULATORY STATUS

Scheduling is a separate determination

We do not claim that Dalargin is unrestricted, non-controlled or approved for U.S. use. Counsel must establish the exact compound and formulation status under federal and state law, with an agency determination where needed. FDA abuse-potential assessment and controlled-substance review are explicit development gates.

07 / PRIORITY DECISION

Why this is not automatically the lead

The ARDS difference is clinically interesting but does not prove a treatment effect. Confirm study quality and resolve shock safety and delivery before selecting a field product. Bemitil remains the focused financed experiment. Dalargin is the highest-priority parallel trauma review.
